Apparently despite our copy-pasting technique we stayed ahead of Google.
So much for adding a bunch of useless information in the code of themes.

Please check the available documentation and search the forums before starting a topic.The information displayed needs to fit on the screen. This is not possible on a mobile device’s very narrow screen. So the columns re-arrange themselves (at specific widths) 2 or 1 per row.
This is intended behaviour to allow the columns to remain at an acceptable size and be readable.

But your issue might not even be WordPress related. All form plugins need some basic server functionality to function (like being able to send an email with the form information).
